Programs & Services


The Queen's Family Health Team provides a wide variety of programs and services to meet the diverse needs of patients. They include:

Anti-coagulation Management Program
 
The Queen’s Family Health Team offers a point-of-care anti-coagulation clinic for patients who are taking anti-coagulation medication (e.g. warfarin, often known as Coumadin). The clinic is coordinated by our pharmacist and is supported by specially trained registered nurses, nurse practitioners and resident family physicians. This team works with patients to monitor lab results, point-of-care results (i.e. blood samples that are collected and processed on-site), adjust medication, and educate patients and their family members.

Breastfeeding  Program

This program provides education, hands-on assistance and support to women and their families regarding infant feeding and nutrition options, both before and  after delivery. 
 
Diabetes Program
 
The Queen’s Family Health Team offers comprehensive care for patients living with diabetes. Our team includes a registered dietitian, a pharmacist, social workers, registered nurses and registered practical nurses, nurse practitioners (several of whom have completed additional training in self-management programming and insulin management), and physicians (including residents).
 
This year, a self-management group program for diabetes is being introduced. Led by specially trained peer leaders (other patients who are living with diabetes), program members will support each other and help build confidence in the management of their health needs. Topics will include: managing symptoms and side effects, managing stress and emotional issues, developing safe exercise and healthy eating plans, managing medications, and developing a strong relationship with your health care team.

Foot Care Program
 
This program is coordinated by a registered practical nurse with specialized training in foot care. She can provide treatments for a wide range of problems of the foot and lower limbs (below the knee), such as corns, calluses (with or without fissures), verrucae (warts), in-grown nails, athlete’s foot, bunions, heel pain and chilblains (perniosis). She can also provide proper nail and skin care to patients with diabetes and other conditions (e.g. those with significant mobility impairments and neurological impairments); basic foot care education including injury risk factors, how to conduct daily self-inspections, how to select proper footwear, and when to seek assistance for issues; and a referral for general orthotics.

Immunization Program
 
Every fall, the Queen’s Family Health Team offers patients a variety of ways to get their annual influenza vaccine. Vaccines are offered during patients’ regularly scheduled appointments; through weekday, evening and weekend clinics; and through home visits for those unable to visit a clinic. We also run vaccine catch-up clinics throughout the year for children.

Lactation Support Program
 
This program is coordinated by an International Board Certified Lactation Consultant who is also a Registered Nurse with additional specialized training and skills in infant feeding.  Her primary role is to provide education, hands-on assistance and support to women and their families regarding infant feeding and nutrition options. Our Lactation Consultant will meet with patients before they deliver (although she will meet with them anytime) and can offer ongoing support after baby is born either in person or over the phone.

Medication Safety Program
 
At the Queen’s Family Health Team, we have a commitment to medication safety. As part of this program, we launched a medication reconciliation initiative in 2008 to help ensure that we always have the best possible record of your medications. Patients are encouraged to bring all of their medications (including over-the-counter medications) to each appointment so that we can make sure that our record includes any changes in dosage or new medications that have been added by a specialist or other provider. At the end of your visit, we will print off a copy of the updated list to take with you when you meet with other providers.
 
This is also a great time to chat with us about any questions you might have about your medication; for example, any side effects you might be experiencing or possible interactions with other medications. We also have a pharmacist available on-site to help with more complex questions.

Smoking Cessation Program
 
In September 2011, the Queen’s Family Health Team launched the Ottawa Model for Smoking Cessation (OMSC), a program developed at the University of Ottawa Heart Institute (UOHI). Members of our clinical team have completed specialized training in smoking cessation counselling and are available to meet with patients to discuss options and strategies for reducing or quitting smoking.
 
As part of this program, patients receive ongoing intervention and follow-up, in person and over the phone. We also provide advice regarding medications (e.g. Champix) and nicotine replacement therapies that might help patients to achieve their goals. As a result of a partnership with the Ministry of Health and Long-Term Care and the Centre for Addiction and Mental Health, we are also able to offer free nicotine replacement therapy to patients enrolled in this program. * You do not need to have a firm quit date before meeting with one of our counsellors. Many patients meet with a counsellor simply to investigate their options and determine when they might be ready to quit.
 
Well-Baby Program
 
Your child’s 18-month mark is an important milestone because he or she is now developing so rapidly, exploring the environment, developing speech and behaviour patterns and, most importantly, building social skills that will last a lifetime. As such, we have implemented an expanded 18-Month Well-Baby Program that focuses on more in-depth developmental and physical assessments, as well as the regular vaccines. 

In particular, this slightly longer visit gives you the opportunity to self-assess your child and learn about his or her progress, and a chance to talk to your provider about things like behavioural issues, communication or motor skills, and community resources that might be helpful. The goal of this program is to ensure that parents and children are supported, and that children are progressing well as they approach school age.

On-site Specialty Clinics:
 
Complementing our programs, our list of on-site specialty clinics includes a Primary Care Asthma Program and a Chronic Pain Clinic. Other services include Procedure Clinics, an Acupuncture Clinic and Pessary Clinic. 
 
Primary Care Asthma Program (PCAP)
 
The PCAP is part of the Asthma Program at Kingston General Hospital. The PCAP Coordinator holds clinics at the Queen’s Family Health Team twice each month. She can provide spirometry (breathing tests), education on proper techniques for taking asthma medications, and overall education on how patients with asthma can better control their symptoms. By helping patients better control their symptoms, patients should experience fewer trips to the emergency room, fewer days away from work or school, and an overall improved quality of life with respect to their asthma.
